Phoenix St. Patrick’s Day Parade - Saturday, March 17, 2012

Registration:	8:30 a.m. – 9:30 a.m. at Monterey Park, 350 E. Oak Street, Phoenix, AZ 85004
Parade starts:	10:00 a.m.
Family Picnic:	11:00 a.m. – 1:00 p.m. Monterey Park in Phoenix 350 E. Oak Street, Phoenix, AZ 85004
Walkers:	Early bird registration ends March 1, 2012. $25 per walker includes a walk t-shirt and entry to Family Picnic After March 1, 2012 $30 per walker includes entry to Family Picnic
Volunteers:	Volunteers receive a free t-shirt and entry to Family Picnic.

In consideration of the acceptance of my entry, I for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages I have or may hereafter have against the organizers of this event, P.O.P.S.I.C.L.E. Center, its principals, its employers, all sponsors and their representatives, volunteers, vendors, and any claims of damages, demands actions whatsoever in any manner, as a result of my participating in the Shamrock Walk event, the P.O.P.S.I.C.L.E. Center Family Picnic, including travel to and from this event. I attest and verify that I am physically fit and have sufficiently trained for the completion of this event and I have not been advised otherwise by a qualified medical person. Further, I hereby grant full permission to use any video or print media of the event without compensation to me. I understand the entry fee and numbers, if any, are non-transferable.
